REGISTRATION FORMS ARE DUE ON OR BEFORE October 15th, 2026

THIS IS NOT THE DATE YOU DROP OFF YOUR QUILT but your entry form MUST be completed and turned in by this date.

YOU MUST INCLUDE A COLOR PHOTOGRAPH OF YOUR QUILT WITH ENTRY.
You may either email a digital picture of your quilt or you can attach a printed picture to your form that you either mail in or drop off in person.

You have 3 options to turn in your form:

1. Drop completed entry form including picture off in person at the Lulu and Hazel Quilt Shop, 502 Bryan Drive Durant, OK 74701, any time on or before OCT 15th, 2026
2. Mail completed entry form including picture to the Lulu and Hazel Quilt Shop, 502 Bryan Drive, Durant OK 74701, on or beforeOCT 15th, 2026
3. Email a copy of completed entry form including picture to entries@quiltfestdurant.com on or beforeOCT 15th, 2026
